MUC1 100-mer Peptide-Pulsed Autologous DC Vaccine |
| Vaccine Information |
- Vaccine Name: MUC1 100-mer Peptide-Pulsed Autologous DC Vaccine
- Target Pathogen: Cancer
- Target Disease: Cancer
- Type: Dendritic cell vaccine
- Status: Clinical trial
- Host Species for Licensed Use: Human
- Antigen: 100-mer MUC1 (Lepisto et al., 2008)
- Immunization Route: Intradermal and subcutaneous (upper arm)
- Description: A synthetic MUC1 100-mer peptide (H2N-(GVTSAPDTRPAPGSTAPPAH)5-CONH2) pulsed autologous dendritic cell vaccine administered intradermally or subcutaneously in the upper arm to 12 patients with resected pancreatic (n=10) or biliary (n=2) cancers. DCs were generated from peripheral blood monocytes via plastic adherence, cultured 6–7 days in IL-4 (1,000 IU/mL) and GM-CSF (1,000 IU/mL), matured with TNF-α, IL-1β, and IL-6 (10 ng/mL each), and pulsed with 100 μg MUC1 peptide for 2–4 hours, yielding 2.0±2.6×10⁶ DC per preparation (mean purity 94±7%). Each dose targeted 1×10⁶ DCs administered at weeks 0, 3, and 6 with a booster at 6 months. The vaccine was well tolerated with no toxicity observed and negative serum ANA tests throughout. No new MUC1-specific IgM or IgG antibody responses were induced post-vaccination, though 3 patients had stable pre-existing responses. Vaccination transiently increased IFN-γ and TNF-α-producing CD4+ and CD8+ T cells and perforin/granzyme B-expressing CD8+ T cells following each dose, returning to baseline thereafter, with no immune parameter distinguishing survivors from non-survivors. Pre-vaccination FoxP3+CD4+ regulatory T cells were significantly elevated versus age-matched healthy controls (p=0.012), increasing transiently post-vaccination before returning to baseline. One long-term survivor showed a sustained ~50% reduction in FoxP3+CD4+ T cells (17.4% to 9.6%). Median overall survival was 26 months (range 13–69 months), with 4 of 12 patients (33%) alive without recurrence at >4 years follow-up (Lepisto et al., 2008).
