- Description: A synthetic peptide vaccine containing a 16-amino-acid MUC1 tandem repeat sequence (GVTSAPDTRPAPGSTA) conjugated to keyhole limpet hemocyanin (KLH) and adjuvanted with DETOX-B. In a clinical trial of 16 patients with recurrent metastatic breast cancer immunized at weeks 0, 2, 5, and 9, all patients generated strong anti-KLH IgG responses (titers 160→5120) but only 3/16 developed weak anti-MUC1 IgG responses (20–80). Following four immunizations, 7/11 evaluable patients (64%) demonstrated class I MHC-restricted cytotoxic T-lymphocyte (CTL) activity against MUC1⁺ HLA-matched adenocarcinoma targets with 18–87% specific lysis. CTL responses were restricted by HLA-A1 (n=4), HLA-A2 (n=2), and HLA-A11 (n=1), demonstrating broad HLA coverage. CD8⁺ T cell-mediated mechanisms were confirmed through antibody blocking studies. Notably, MUC1-specific CTL precursors were detectable after only a single 6-day in vitro restimulation with MUC1 peptide plus IL-12, IL-2, and IL-4, compared to the multiple weeks of restimulation required in previous cancer vaccine studies, indicating successful breaking of immune tolerance. MUC1 is a membrane-bound glycoprotein overexpressed in an aberrant or underglycosylated form in breast adenocarcinomas and other epithelial cancers including ovarian, pancreatic, and colorectal malignancies (Reddish et al., 1998).
