  • Description: A synthetic 100-amino acid MUC1 peptide vaccine (H2N-(GVTSAPDTRPAPGSTAPPAH)5-CONH2) representing five tandemly repeated 20-amino acid epitopes, adjuvanted with SB-AS2 (MPL + QS-21), administered intramuscularly every 3 weeks for three doses at escalating peptide doses (100, 300, 1,000, and 3,000 μg) in 16 patients with resected or locally advanced pancreatic cancer. The vaccine was well tolerated with no grade 3 or 4 toxicities; adverse effects were limited to grade 1 flu-like symptoms (25%), injection site tenderness (38%), and grade 1–2 erythema (37%). In 10/16 patients, T-cell cytokine production (IFN-γ and IL-4) was restored postvaccination, most striking in 5/16 patients. CD8+ T cell frequencies increased postvaccination, shifting the CD4+:CD8+ ratio from the expected 2:1 to CD8+-predominant. CD8+ T cells showed increased IFN-γ production and TCR ζ chain expression (p=0.03), with preferential infiltration at MUC1 DTH sites. MUC1-specific IgG responses were observed in 4/15 patients, with no dose-response relationship across the 30-fold dose range. Median survival was 12 months; 2/15 resected patients remained disease-free at 32 and 61 months (Ramanathan et al., 2005).
